29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: Energy including, but not limited to pneumatic, was not released or shut down while an employee attempted to clear a jam on a tray transfer machine on N. Tullip, Line 9. On or about January 23, 2015, an employee suffered a crushed finger tip injury that resulted in an amputation from placing his hand in an associated danger zone on an energized tray transfer machine. An alternate measure to safely clear the jam was also not attempted.
