29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii).   On or about January 29, 2015, an employees was observed painting a commercial building roof flashing while working from a frame scaffold platform that was not fully planked, exposing employees to fall hazards exceeding 10 feet.

29 CFR 1926.451(c)(2)(ii): Unstable objects were used to support scaffolds or platform units.  On or about January 29, 2015, and times prior thereto, an employee was observed on a frame scaffold platform painting the building roof flashing with heights above 10 feet from the ground, where the scaffold baseplates were supported by unstable mudsills, and uneven ground.  This condition made the scaffold unstable and exposed employees to fall hazards.

29 CFR 1926.451(e)(8): Direct access to or from another surface was used when the scaffold was more than 14 inches (36 cm) horizontally from the other surface:  On or about January 29, 2014, and at times prior thereto, an employee was stepping from one scaffold working platform to another when the distance between them where more than 14 inches apart at heights of above 10 feet from the ground.  This condition expose employee to fall hazards.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews.  On or about January 29, 2015, employees working on a fabricated frame scaffold were not protected from falling bya guardrail system, exposing them to falls exceeding 10 feet.

On or about January 29, 2015, and times prior thereto, an employee was observed on a frame scaffold platform painting the building roof flashing at heights above 10 feet from ground that was not provided with toe boards.  This condition exposed employees to struck-by hazards.

29 CFR 1926.452(c)(2): Frames and panels were not braced by cross, horizontal, or diagonal braces, or combination thereof, which would secure vertical members together laterally, and the cross braces were not of such length as would  automatically square.   On or about January 29, 2015, and times prior thereto, two employees were observed on a frame scaffold platform painting the building roof flashing and installing wall insulation at heights above 10 feet from ground were some of the vertical panels were not provided with cross braces as to automatically square the panels. This condition exposed employees to fall hazards.
