The employees were exposed in that directional exit signs were not immediately apparent to direct them to the nearest exit and from keeping them from accidently entering a room that may not lead to an exit the areas known as:  a) Accounting Office  b) HR Office  c) Executive Area  d) Sales Department  f) Call Center Department  g) BSG Room

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place:  Employees operating forklifts in the facility shop area were exposed to struck-by hazards. The employees were exposed in that the control levers which moves the forks were missing the markings to indicate which direction the operator should move the lever(s) to avoid moving it in the opposite direction and striking someone or something on the:  a) Nissan Forklift, 15,500lb Capacity, Model: DF05N70V, Type D, Chasse #DF05-00315, Marked #3800  b) Linde Forklift Model H30D, Weight 10329, Type D, SN # H2X393B01583  DEFFENBAUGH INC. WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.178(a)(6), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 620880 , CITATION NUMBER 1, ITEM 1, ISSUED ON 12/18/2012, WITH THE FINAL ORDER DATE OF 01/15/2013, WITH RESPECT TO A JOB SITE LOCATED AT 2402 S. 88TH ST., KANSAS CITY, KS 66111.
