29 CFR 1910.178(n)(4): Industrial truck driver(s) were not required to travel with the load trailing whenever the load obstructed forward view: (a) Warehouse: Pedestrians were exposed to struck by hazards when a forklift operator was driving forward with a load obstructing the forklift operator's view. On or about January 6, 2015, an employee was injured after being struck by a forklift which was being driven forward with the insulation blocking the operator's view.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): (a) Warehouse: Employees were exposed to struck by or caught between hazards as the Clark Clamp Forklift Model CGC 20 had no horn and the brakes needed to be replaced. The forklift was not taken out of service until it could be restored to a safe operating condition. (b) Warehouse: Employees were exposed to struck by or caught between hazards as the Clark Clamp Forklift Model CGC-25 required a new horn button, and the back up alarm was not repaired. The forklift was not taken out of service until restored to a safe operating condition.
