29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: In an aerial lift, an employee was observed in a JLG lift without personal fall arrest system lanyard anchored to the boom or basket. This exposed the employee to not being able to protect himself from being bounced out/off of the basket/work platform when maneuvering to a work location.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. An employee working from a JLG Lift Sky Power 460si boom lift was exposed to an 18 foot fall from elevation hazard had not been trained to the hazards associated with the use of the equipment.
