29 CFR 1904.32(b)(2): The Summary of Work-Related Injuries and Illnesses (OSHA Form 300A or equivalent) was incomplete:     a) On or about February 13, 2015, at the above located workplace, the employer's OSHA 300A form for 2014 was not completed in detail as required by 1904.4 through 1904.32.   In that:     i) The summary listed 6 cases with job transfer or restriction injuries and no restricted work days.  ii) The summary did not include the date the 300A form was certified by the company Vice president.

29 CFR 1910.303(g)(2)(i):  Electrical equipment operating at 50 volts or more was not guarded against accidental contact by approved cabinets or other forms of approved enclosures:   a) On or about January 28, 2015, at the above located workplace, employees working at the old take-up reel machine were required to work with and around a push button control box.  The 120 volt control box was bent and partially open. The energized (120 volt) wiring in the box was not guarded by an approved cabinet or other approved enclosure. Employees were thereby exposed to hazards associated with electricity.    b) On or about January 28, 2015, at the above located workplace, employees working at the old take-up reel machine were required to work with and around 230 volt Dayton drive motor. The flexible conduit containing the wires to the motor was not connected to the motor. The lack of a connection between the conduit and the motor prevented the conduit from 1) functioning as a path to ground and 2) functioning as a strain relief. The 230 volt wires were not guarded by an approved cabinet or other approved enclosure. Employees were thereby exposed to hazards associated with electricity.
