29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE):     a)  On or about January 18, 2015, during routine preventative maintenance on the Compac Rotary Valve on top of the Balemaster, Model # 4075G-8, Serial #12316B, located in Plant B, the employer did not assess the workplace to determine hazards that necessitate the use of personal protective equipment for falls and struck by hazards approximately 4 feet 5 inches above the ground.     b) On or about January 26, 2015, during servicing operations on the Flexo Emba 245 cm FFG/Diecut Prefeeder Quick Set, Machine #1415, located in Plant C, the employer did not assess the workplace to determine hazards that necessitate the use of personal protective equipment for falls and struck by hazards approximately 5 feet 5 inches above the ground.     c) On or about January 26, 2015, during servicing operations on the AFP Machine On the Module Facer, located in Plant B, the employer did not assess the workplace to determine hazards that necessitate the use of personal protective equipment for falls and struck by hazards approximately 9 feet 3 inches above the ground.     e) On or about January 26, 2015, during preventative maintenance operations on the Air Converting Corporation East blower Mezzanine, located in Plant B, the employer did not assess the workplace to determine hazards that necessitate the use of personal protective equipment for falls and struck by hazards approximately 13 feet 4 inches above the ground.
