29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a) On or about 1/27/15 at the Stratford Apartments construction project locate at 4505 Chestnut Ridge Road, Amherst, New York; in the office trailer, a door with change in elevation of 36 inches was not provided with stairs or barricaded to prevent use. An employee was injured when the door was used as an exit from the trailer and the employee fell to the ground. NO ABATEMENT CERTIFICATION REQUIRED
