On or about January 22, 2015, employees were repairing and maintaining an elevated Presort Conveyor (approximately 38 inches above the platform), which is not a walking or working surface, which was locked out, and the chute opening (approximately 38 inches above the platform), which is adjacent to the conveyor and used for employee to sort materials while standing on the platform, did not have any guards or covers to protect employees from falling to lower levels, if they were working on top of the conveyor.
