29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet (1.22 m) or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides:    At the time of the inspection, the employee priming the top of the ambulance was standing on the top of the ambulance with a fall to the floor of greater than 7 feet. No guard rail system or fall protection was provided.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:    At the time of the inspection, the employer had not certified that a hazard assessment had been conducted. Employees were supplied personal protective equipment such as gloves, supplied air respirators, dust masks, 1/2 mask respirators, protective clothing and hearing protection.

29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use:    At the time of the inspection, painters were required to wear supplied air respirators and the employer had not developed a written respiratory protection program. The employer also supplied Moldex dust masks to employees exposed to body filler dust.
