Protection by placing and keeping such materials or equipment at least 2 feet from the edge of excavations, or by the use of retaining devices that are sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary was not provided: (a) On January 28, 2015, at worksite 7904 Park, Kansas City Missouri.- employees working in an 6-8 ft excavation was exposed to serious struck by and or engulfment hazard, in that the spoils piles had not been placed at least two feet from the edge of the excavation. Abatement certification for this violation is not required.
