29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: The employer does not ensure that a body belt with a lanyard attached to the boom or basket is worn by employee(s) working from an aerial lift. This violation was observed on or about January 29, 2015, on the south side of the commercial building where employees were exposed to a fall hazard when applying stucco to the exterior from an aerial lift (SN: JA07026) without a fall protection system. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that that each employee working from an aerial lift is wearing a body belt with lanyard attached to the boom or basket.

29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: The employer does not ensure that the path to ground from circuits, equipment, or enclosures is permanent and continuous. This violation was observed on or about January 29, 2015, at the worksite of the commercial building where employees were using a Dewalt electric drill that was missing the grounding pin. Pursuant to 29 C.F. R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the path to ground from circuits, equipment, or enclosures is permanent and continuous.
