The employer does not ensure that supported scaffolds with a height to base width ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, or bracing. This violation was observed on or about January 29, 2015 when employees were exposed to a fall hazard when working from a tubular welded frame scaffold whose height to base width ratio was more than 4:1 and was not restrained from tipping. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees working from supported scaffolds with a height to base width ratio of more than four to one (4:1) are restrained from tipping by guying, tying, or bracing.

This violation was observed on or about January 29, 2015 when employees were exposed to a fall hazard when accessing a tubular welded frame scaffold without adequate access.
