29 CFR 1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe condition(s) and the regulation(s) applicable to his work environment to control or eliminate any hazard(s) or other exposure to illness or injury.       (a)  Hydro Construction Company Inc. at 1500 New Liberty Road, Windsor, CO 80550: On and before January 22, 2015, the exposing employer did not ensure that a foreman and two employees were instructed in the recognition and avoidance of unsafe conditions associated with the un-banding of steel concrete supports under pressure at the construction site. Employees were engaged in manually preparing concrete supports into bundles for transportation and which in some instances required the complete un-banding of the bundles by the employees before restacking and re-banding them. One of the workers cut the last band on a bundle of supports and it sprung loose and struck the employee resulting in his hospitalization. This condition exposed employees to a struck by and crushing hazard.
