Baler M-72 was not operating properly which allowed the compression head to lower when it was not in use and raise without the safety door shut in an automatic mode. b) On or about 2/2/2015, the Cup Packaging Machine had two nip points that needed guarding. One was for the sealer head and the second was for the lid feeder. c) On or about 2/2/2015, the Funnel Agitator was turning with the lid open, exposing employees to caught-in injuries.
