29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a)    On or about 1/30/2015, the VER-TECH, Inc. Baler M-72 was not operating properly which allowed the compression head to lower when it was not in use and raise without the safety door shut in an automatic mode.  b)    On or about 2/2/2015, the Cup Packaging Machine had two nip points that needed guarding. One was for the sealer head and the second was for the lid feeder.  c)    On or about 2/2/2015, the Funnel Agitator was turning with the lid open, exposing employees to caught-in injuries.
