29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: a) Englewood residence: On or about January 26, 2015, and at times prior, a worker without a hardhat was exposed to head injuries from falling debris and tools while working at ground level adjacent to overhead roofing work. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, nor was each employee provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Englewood residence: On or about January 26, 2015, and at times prior, seven workers were not wearing a harness and had no other means of fall protection while exposed to falling 20.5 feet to ground level. b) Englewood residence: On or about January 26, 2015, and at times prior, a worker without fall protection was exposed to falling 8 feet to concrete from the bed of a truck loaded with roofing debris. Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).
