The most recent occurrence was at 815 Victory Drive, St. Louis, MO 63125, where they failed to make frequent or regular inspections of the worksite to ensure all workers were in compliance with company safety and health policies and OSHA standards. The workers were exposed to a fall hazard ranging from approximately 9 feet to 10 feet as they were performing residential roofing activities on an approximate 3:12 pitch without the use of any fall protection system. In addition, employees failed to wear eye protection while operating nail guns to perform roofing operations.
