OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushed-by hazards created by the use of industrial storage racks which were not anchored to the floor: a) Warehouse area, 6009 Enterprise Drive, Export, PA 15632: Observed on or about January 16, 2015, employees were using industrial storage racks that were not anchored to the floor. The industrial storage racks were used to store equipment and pallets of materials. The industrial storage racks were at risk of tipping over while being loaded or unloaded with powered industrial trucks. Employees loading the industrial storage racks and employees working in the area of the industrial storage racks were exposed to crushed-by and struck-by hazards.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that an employee was exposed to injuries from a fall hazard of up to approximately 20 feet to the concrete floor below while using an evated work platform that was not secured to the lifting carriage and/or forks of a forklift: a) Warehouse area, 6009 Enterprise Drive, Export, PA 15632: On or about February 4, 2015: An employee was working from a work platform that was not firmly secured to the lifting carriage and/or forks of a Hyster brand powered industrial truck (model number: E120XL, serial number C09BD03563U) through the use of a safety chain.

29 CFR 1910.305(a)(2)(ix): All lamps for general illumination were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard: a) Warehouse area, above surge bin, 6009 Enterprise Drive, Export, PA 15632: Observed on or about January 16, 2015, the light bulbs on a light fixture located above a Conair brand surge bin were not protected from accidental contact or breakage by a suitable fixture or lampholder with a guard. Employees unclogging the surge bin while standing on a step ladder worked within approximately 3 feet of the light fixture. The employees were exposed to the hazards of electric shock and cuts and scrapes from broken glass in the event that the light bulbs would break.
