29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) (a)Mario Padilla Construction, LLC at 2528 Walnut Street, Denver, CO 80205: On and before January 30th, 2015, the exposing employer did not ensure two employees were protected from fall hazards while working on the same floor where an open sided floor hole existed. The employees were engaged in framing operations on the second level of structure under construction. This condition exposed employees to a fall hazard of approximately 14 feet
