Worksite - The employer did not establish a lockout or tagout program consisting of specific machine hazardous energy control procedures, training for employees and a periodic inspection of established procedures for the Power Presses, Lathes, Milling Machines and Surface Grinding Machines thereby exposing employees to potential hazardous energy release during maintenance and servicing of equipment.

Building 4 - The soapstone belt sanders belt and pulley was not guarded or covered leaving employees exposed to in running nip points between the belt and pulley. b. Building 4 - The rotating shaft of the drill chuck on the free standing drill press machine I-26 was not covered or guarded thereby exposing employees to the rotating parts on the chuck. c. Bending Area - The rotating chuck and shaft on the Do-All drill press (DTR-28, SN 65-8705) were not guarded thereby exposing employees to rotating parts on the chuck. d. Lathe Room - The rotating chick on the LeBlond lathe (SN 113-906) was not covered or guarded thereby exposing the employees to a rotating parts hazard. e. Horizontal Boring Area - The Shibaura Machine (BT-10B) rotating parts were not covered or guarded thereby exposing employees to caught by hazards. f. Horizontal Boring Area - The Shibaura Machine (BT13C) rotating parts were not covered or guarded thereby exposing employees to caught by hazards.
