29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section. On or about 27 January 2015, and at times prior thereto, pouring operators were allowed to inspect and clean the strainer/filter on the PP-22 Tank Line without a developed and documented energy control procedure, exposing the employees to a sudden release of approximately 2200 gallons of 50% mixture of toluene diisocyanate (TDI) from the line and tank.

29 CFR 1910.147(d)(4)(i): Lockout or tagout devices were not affixed to each energy isolating device by authorized employees. On or about 27 January 2015, and at times prior thereto, pouring operators were allowed to inspect and clean the strainer/filter on the PP-22 Tank Line without the equipment being locked out, exposing the employees to a sudden release of approximately 2200 gallons of a 50% mixture toluene diisocyanate (TDI) from the line and tank.

29 CFR 1904.7(b)(4): When an injury or illness involved restricted work or job transfer, the OSHA 300 Log of recordable work-related injuries and illnesses did not have an entry of the total number of restricted or transferred days in the job transfer or restricted work days column. At the inspection site, the OSHA Form 300 Log of Work-Related Injuries and Illnesses for calendar year 2014 was not completed correctly in that column L (number of days the injured employee was on the job transfer or restriction) was not entered when column I (classifying the case as job transfer or restriction) was marked for the same reported case. Cases 2701, 2762, and 2965 had the column I marked but had no corresponding number entered in column L.
