29 CFR 1910.138(a): Employees were not wearing appropriate hand protection when they were exposed to hazards such as those from skin absorption of harmful substances; severe cuts or lacerations; severe abrasions; punctures; chemical burns; thermal burns; and harmful temperature extremes: (a) Grooming Salon - Employees using "Sentry Flea and Tick Shampoo for Dogs And Puppies" did not wear gloves, on or about January 30, 2015. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records. PETSMART INCORPORATED WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH ADMINISTRATION STANDARD OR IT'S EQUIVALENT STANDARD 29 CFR 1910.138(a)(1) WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 315888560 CITATION NUMBER 1, ITEM NUMBER 1a, AND WAS AFFIRMED AS A FINAL ORDER ON JULY 27, 2012, WITH RESPECT TO A WORKPLACE LOCATED AT 1183 BERKSHIRE BOULEVARD, WYOMISSING, PA 19610.

29 CFR 1910.1200(h)(1): Employers did not provide employees with effective information and training as specified in 29 CFR 1910.1200(h)(2) and (3) on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard was introduced into their work area: (a) PetSmart, Incorporated - The employer did not provide information and training on chemicals such as but not limited to "Sentry Flea and Tick shampoo for Dogs and Puppies," hydrosurge face wash and furminator deshedding solution, on or about January 30, 2015. ABATEMENT NOTE: Employees shall be informed of the following: 2. The requirements of this section; 3. Any operations where hazardous chemicals are present; 4. The location and availability of the written Hazard Communication Program, list(s) of hazardous chemicals, and Material Safety Data Sheets. Employee training shall include at least: 1. Methods and observations that may be used to detect the presence or release of a hazardous chemical in the work area; 2. The physical and health hazards of the chemicals in the work area; 3. The measures employees can take to protect themselves such as specific procedures and personal protective equipment to be used; 4. The details of the employer's Hazard Communication Program including an explanation of labeling system, Material Safety Data Sheets, and how employees can obtain and use the appropriate hazard information.
