Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by hazards: a) On or about January 21, 2015 the employer the employer used the drilling rigs rotary clutch system powered by the floor engines to apply counter clockwise torque on the drill pipe that was stuck in the hole. The clutches were operated at a torque greater than the maximum torque limitations as specified by the manufacturer.
