29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   A.)  Raymundo Martinez Stucco; Taco Bell; 1990 E. State Street, Hermitage, PA - On or about 10/28/14, and at times prior thereto, employees were not wearing hard hats while exposed to overhead hazards from falling objects.  Employees were working on the ground and from a scaffold while roofing work was taking place above them.  Sources of falling objects included but were not limited to, tools, and materials utilized in the roofing operations.

Raymundo Martinez Stucco; Taco Bell; 1990 E. State Street, Hermitage, PA - On or about 10/28/14 and at times prior thereto, a competent person was not deployed on the job site to perform frequent and regular inspections.  Safety deficiencies at the job site included but were not limited to, scaffolds, electrical hazards, and personal protective equipment.

29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and used in accordance with instructions included in the listing, labeling, or certification:   A.)  Raymundo Martinez Stucco; Taco Bell; 1990 E. State Street, Hermitage, PA - On or about 10/28/14 and at times prior thereto, a knockout style receptacle outlet box was hardwired onto the end of an extension cord and utilized as a cord cap.  The knockout style receptacle outlet box was not designed to be utilized in portable applications, outside exposed to weather and damp/wet locations, and in a construction work environment.  The knockout style receptacle outlet box was being utilized to power an orange extension cord.

29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables used for temporary wiring were not protected from damage.  Protection was not provided to avoid damage when flexible cords and cables passed through doorways or other pinch points:    A.)  Raymundo Martinez Stucco; Taco Bell; 1990 E. State Street, Hermitage, PA - On or about 10/28/14 and at times prior thereto, a red/black colored extension cord was routed form the interior of the building and underneath the front door of the building to a work location on the outside of the building.  The area where the extension cord was routed underneath the door was not protected from physical damage.  The extension cord was utilized to power a knockout style receptacle outlet box.

State Street, Hermitage, PA - On or about  10/28/14 and at times prior thereto, an orange colored extension cord was frayed near the cord cap.  The outer jacket of the cord was pulled from the cord cap.  The extension cord was utilized to power a handheld portable grinder.

Raymundo Martinez Stucco; Taco Bell; 1990 E. State Street, Hermitage, PA - On or about 10/28/14 and at times prior thereto, employees were conducting work operations from a frame scaffold.  The work operations included but were not limited to stucco work.  The scaffold's working deck was not completely planked and/or fully decked.  The scaffold's working deck was only outfitted with one plank.
