29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazards created by rotating shafts: a) Johnson Leesona Corporation Re-Winder (Control Box No. 13) - Machine operators working near a rotating shaft, particularly while positioning themselves to remove a completed reel of nylon film from a second shaft are exposed to a caught-in hazard posed by the rotating shaft having an unsmooth surface due to a milled keyway. Condition observed on January 28, 2015. Pursuant to 29 CFR 1903.19, the employer must submit abatement certification, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed: a) Ensinger Inc. DBA Penn Fibre Plastics, Inc., Greenwood, DE: The employer did not conduct an annual evaluation of the energy control procedures to ensure the maintenance personnel are following the procedures. The employees work in the area with the equipment on a daily basis. Observed on or about January 28, 2015. Penn Fibre Plastics, Inc. was previously cited twice for a violation of this occupational safety and health standard, or its equivalent standard 1910.147(c)(6)(i), which was contained in: OSHA Inspection Numbers 311813653, Citation Number 1, Item Number 1, issued on 4/19/2012 and affirmed through an informal settlement agreement signed on 5/1/2012; and, OSHA Inspection Number 311805642, Citation Number 2, Item Number 3, issued on 03/23/2009. Pursuant to 29 CFR 1903.19, the employer must submit abatement certification and documentation, required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
