29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    a)  On or about January 30, 2015, and at times prior, alpine guide employees were exposed to falls because the employer did not adequately enforce the use of ropes, anchors, and other devices to provide adequate fall protection during ice climbing activities.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification that identifies the workplace evaluated, the person certifying that the evaluation has been performed, the date(s) of the hazard assessment, and, which identifies the document as a certification of hazard assessment:    a)  On or about January 30, 2015, and at times prior, alpine guide employees were exposed to falls because the employer did not develop a complete hazard assessment and certify such written hazard assessment which evaluated the hazards present for guides and determined the type of fall protection and other personal protective equipment required to be used by the guides.

29 CFR 1904.32(a)(1): The employer did not review the OSHA 300 Log to verify that the entries are complete and accurate, and correct any deficiencies:    a)  On or about February 3, 2015, and at times prior, the employer did not review the OSHA 300 Log to verify accuracy of entries and correct deficiencies.  A back injury which occurred on December 29, 2014, and involved 2 days away from work was not recorded on the OSHA 300 Log for 2014.
