29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 22, 2015 and times prior thereto, employees of Mets Enterprises, LLC performed maintenance and servicing on machinery used in the production of sandwich wraps and bags for the food service industry. The employer had not documented procedures for the control of potentially hazardous energy, exposing employees to electrical and caught-in hazards.

29 CFR 1910.147(f)(1)(i): 29 CFR 1910.147(f)(1): When lockout or tagout devices were temporarily removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions in 29 CFR 1910.147(f)(1)(i) through (f)(1)(v) were not followed: In the alternative: 29 CFR 1910.147(f)(1)(ii): When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, the employees were not removed from the machine or equipment area in accordance to the paragraph (e)(2): On or about January 22, 2015 and times prior thereto, employees of Mets Enterprises, LLC performed maintenance and servicing to machinery manufacturing sandwich wraps and bags for the food service industry located at or near 167 Airport Road in Salem, Arkansas. During adjustments for laminator #1, lockout devices were temporarily removed with the equipment energized to observe the web of paper for wrinkling. The employer had not removed materials and employees from the machine area during this operation, exposing employees to caught-in hazards.
