29 CFR 1926.95(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided:    (a)Electric Martinez, LLC at 3255 North Williams Street, Denver, CO 80205:  On and before February 3rd, 2015, the employer did not ensure an employee performing voltage testing to work on energized circuits without the use of rubber insulating gloves.  The condition exposed employees to 120 volt electrical shock hazards.

29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means:    (a)Electric Martinez, LLC at 3255 North Williams Street, Denver, CO 80205:  On and before February 3rd, 2015, the employer permitted an employee to wire a kitchen light fixture while using only a kitchen light switch to de-energize the circuit.  The employee did not de-energize the circuit at the breaker, and did not test the circuit prior to the work to ensure the circuit was de-energized.  The employee was not using rubber insulating gloves.  The three kitchen light switches on this circuit were not secured to the electrical box, the box was missing its faceplate, and the three switches had exposed copper wire.  This employee and a second employee were not trained to de-energize, to lock and tag, and to test the circuit prior to use.  The condition exposed employees to 120 volt electrical shock hazards.

The employer permitted an employee to use the top step of a stepladder as a step.  The conditions exposed employees to fall hazards of up to 6 feet.
