Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems.  On 2/3/2015, workers from Link Construction, LLC were installing shingles on a roof of a two-story apartment building located on Sanchos Circle in OFallon MO.  They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems.  The foreman and two employees were observed on the roof of the building without fall protection, exposing them to a fall to the ground below.

On 2/3/2015, workers from Link Construction, LLC were installing shingles on a roof of a two-story apartment building located on Sanchos Circle in OFallon MO.  They were using a ladder improperly to access a second story landing.  The ladder was not extended to three feet beyond the landing surface.
