29 CFR 1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner: (a) Jobsite: On or about January 29, 2015, two employees were working from a mobile scaffold and none of the four wheels were locked while they were working from it. The fall hazard was about 5 feet 9 inches to the concrete floor.
