Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to caught by hazards:   a. On or about January 27, 2015, at the shop area of the above addressed worksite, an employee was exposed to a caught by hazard while cutting metal with a radial saw (i.e. Delta, no model number available) that was not adjusted to prevent the forward travel of the blade beyond the position necessary to complete the cut and its cutting head did not return gently to the starting position when released by the operator.

On or about January 31, 2015, at the shop area of the above addressed worksite, an employee was exposed to a caught by hazard while making holes on a piece of metal with a drill press (i.e. Jet Serial No. 9020452) that lacked a guard on its point of  operation.    b.  On or about January 22, 2015, at the shop area of the above addressed worksite, an employee was exposed to a caught by hazard while making holes on a piece of metal with a drill press (i.e. Dayton Model 5PHCA) that lacked a guard on its point of  operation.    c.  On or about January 27, 2015, at the shop area of the above addressed worksite, an employee was exposed to a caught by hazard while cutting metal with a radial saw (i.e. Delta, no model available) that had the lower portion of the blade unguarded.
