In the framing shop, employees were exposed to caught in and shock hazards ,in that, while conducting maintenance on the Commander DI SN# 04-134-002 and Patriot SN# 05-23-003 door frame making machines no lock out tag out program was in place to control hazardous energy, on or about 02/03/2015.
