29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: a- Bombardier Transportation (Holdings) USA, Inc., Newark, NJ - The LO/TO procedures in place did not clearly and specifically outline procedural steps when performing maintenance on the guideway system. The procedures did not specifically address isolating, blocking and securing machinery and equipment when employees are required to walk on or near an operational active guideway while maintenance work is being performed done between stations. Condition noted on or about 1/30/15.
