29 CFR 1926.451(f)(14): Makeshift devices, such as but not limited to boxes and barrels, were used on top of scaffold platforms to increase the working level height of employees: a. On or about February 3, 2015, at the above addressed jobsite, an employee working from a scaffold was exposed to a fall hazard as he step on a concrete block to increase his working height approximately 6 feet from the ground level.

29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section: a. On or about 02/03/2015, at the above addressed jobsite, employees were exposed to a fall hazard of approximately 15 feet to the ground level as they worked from a tubular welded scaffold without fall protection.
