29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:      On or about February 4, 2015, the employer failed to ensure that 480v electrical panels located in the welding department of the facility were provided with sufficient access and working space. The panels were blocked by and industrial fan, industrial welder, and miscellaneous equipment.

29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy.    On or about February 4, 2015 the employer failed to ensure that the energy control procedures for the 150 ton Verson Bed 36x60 Press (#29) specifically outlined the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy. The employer's energy control procedures did not identify all potential energy sources to include but not limited, gravity of the dies on mechanical power presses, and to ensure that die blocks are used for blocking/ securing the upper die.         The Falls Stamping and Welding Co. Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii)(b), which was contained in OSHA inspection number 769542, citation number 1, item number 1a and was affirmed as a final order on 04/22/2013, with respect to a workplace located at 1720 Fall Street, Cleveland, OH.

29 CFR 1910.147(c)(4)(ii)(D): The energy control procedures did not clearly and specifically outline the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control    On or about February 4, 2015 the employer failed to ensure that machine specific procedures of the 150 ton Verson Bed 36x30 mechanical power press (#29) clearly and  specifically outlined the requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control. The employer's energy control procedure did not include enough specificity which included using die blocks to secure the upper arm and to include verification that the press was de-energized when other maintenance was conducted on the press(es).      The Falls Stamping and Welding Co. Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(ii)(b), which was contained in OSHA inspection number 769542, citation number 1, item number 1b and was affirmed as a final order on 04/22/2013, with respect to a workplace located at 1720 Fall Street, Cleveland, OH.

29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:    On or about February 5, 2015, the employer failed to conducted a periodic inspection of the energy control procedures at least annually. Equipment and machinery within the facility includes but is not limited to, presses, grinders, straighteners, cut-off saws, and cranes. Potential hazardous energies include pneumatic, electrical, mechanical, hydraulic, and gravitational.    The Falls Stamping and Welding Co. Inc was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(6)(i), which was contained in OSHA inspection number 106894, citation number 1, item number 3 and was affirmed as a final order on 10/15/2012, with respect to a workplace located at 1720 Fall Street, Cleveland, OH.
