Employees did not control electrical hazardous energy source while conducting activities such as, but not limited to, cleaning the roller. Employees were exposed to amputation hazards.

Employees were exposed to injuries from unexpected start-up of the machine and energy release.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts: A) Hearthside Food Solutions, LLC, located at 312 Rader Road, McComb, OH 45858: On or about January 27, 2015, the employer did not to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, moving slide of the Go-Cup machine (manufactured by Packaging Technologies, Model # PR-4, Serial # 15350). Employees were exposed to struck-by and amputation hazards. B) Hearthside Food Solutions, LLC, located at 312 Rader Road, McComb, OH 45858: On or about November 25, 2014, the employer did not to ensure that one or more methods of machine guarding were provided to protect employees from nip points between the belt and the rollers, as well as from rotating rollers of the Line 13 incline feed conveyor. Employees were exposed to amputation hazards.
