29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. At the workplace, at press A-3, the ingoing moving feed bar was not adequately guarded, thereby exposing employees to a caught-in hazard. b. At the workplace, at press K-9 coil feed end, employees were not guarded from the caught-in hazard between the ingoing coil and press die. On January 23, 2015, an amputation occurred from the unguarded caught-in hazard between the ingoing coil and press die.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. a. At the workplace, between press 5 and 6 on RY-1, a metal outlet box that was powering an industrial fan had flexible cords attached that were not connected to a device and fitting so that strain relief was provided, thereby exposing employees to a electrical hazard.
