29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:   a) Roof (Unit #500), Sutter's Point Townhomes located at 6000 Glenridge Drive NE, Sandy Springs, GA 30328: Employees were exposed to falls of approximately 18-feet, 1-inch while engaged in roofing activities on a 5/12 pitched roof with no fall protection system in place.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  (a) Front side (unit #500), Sutter's Point Townhomes located at 6000 Glenridge Drive NE, Sandy Springs, GA 30328: The ladder provided was not secured to the structure and did not extend at least 3 ft. above the working surface to allow for safe access on and off the roof. Employees accessing this ladder were exposed to falls of approximately 18-feet, 1-inch to the lower level.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  (a) Front side (unit #500), Sutter's Point Townhomes located at 6000 Glenridge Drive NE, Sandy Springs, GA 30328: The employer failed to provide a ladder which was free from damage as the top rung was bent and the side rails were twisted at the top and not removed from service. Employees using this ladder were exposed to possible collapse and falls hazards as the strength of the ladder has been compromised by the damage.
