29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition. a) On or about 02/04/2015 at the end of the warehouse - employees were exposed to a slip and fall hazard, in that, there was petroleum fluids on the floor where oil and hydraulic drums/tank were stored.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition(s): a) On or about 02/04/2015 in the company's warehouse areas employees were exposed to struck-by hazards and fire hazards, in that, two forklifts were being used with defective parts; 1. A Hyster 80 #743 forklift vehicle did not have a serviceable horn. 2. A Yale #739 forklift vehicle did not have a serviceable horn.
