29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Production: Employees were exposed to amputation hazards as the Windmoller & Holscher D454 Press did not have adequate point of operation guards as the opening was approximately a foot between the two plexiglass sheets at the print stations. (b) Production: Employees were exposed to amputation hazards as the sides of the Windmoller & Holscher D454 Press did not have point of operation guards to keep employees from reaching into the ingoing nip points from the rotating cylinders.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area: (a) Throughout Facility: All production employees exposed to chemicals were not trained at the time of their initial assignment. Employees were exposed to chemicals such as ID-55 which contains potassium hydroxide that can cause burns to skin and eyes; and E-Chem 901 that is a solvent that contains Xylene and is flammable.
