29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about January 26, 2015, at the oven transfer line number 5, in the Tyson Foods facility in Pine Bluff, Arkansas, an employee attempted to clear a jam in the intake of a Uniblock PD 400 pump. The employer had not required employees to utilize procedures for the control of hazardous energy. These conditions exposed the employee to the hazard of being caught in the pump's impeller.

29 CFR 1910.147(c)(7)(i)(B): Each affected employee was not instructed in the purpose and use of the energy control procedure: On or about January 26, 2015, an affected employee, who had not been instructed in purpose and use of the energy control procedure, attempted to clear a jam, in the intake of a positive displacement pump, that had not been shut down or locked out, these conditions exposed the employee to the hazard of being caught in the pump impeller.
