29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: On or about February 2, 2015, at the job site located at 144 Creekside Drive in Austin, Arkansas, employees who were at ground level, below an employee working on the roof, were not wearing protective helmets. These conditions exposed the employees to the hazard of being struck by falling objects.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about February 2, 2015, at the job site located at 144 Creekside Drive in Austin, Arkansas, employees installing decking and tar paper on the roof of the new residence were not using any form of fall protection. These conditions exposed the employee to the hazard of falling approximately 11 feet to the ground below.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about February 2, 2015, at the job site located at 144 Creekside Drive in Austin, Arkansas, an employee who was working on the roof of the new residence used an extension ladder to access the roof. The ladder's rails did not extend at least 3 feet above the upper landing. these conditions exposed the employee to the hazard of falling approximately 11 feet to the ground below.
