29 CFR 1910.178(l)(4)(i): Refresher training in relevant topics was not provided to the operator who had been involved in an accident or near-miss incident: (a) On or about February 2, 2015, at the above addressed jobsite, an employee who had not received adequate training, was operating a powered industrial truck while travelling forward with a load on the forks causing an obstructed the view of the path of travel, collided with an approaching tug tractor, and an employee who was observed travelling with the forks of the lift raised about 36 inches.
