29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied:  Employees at a residential construction job at 5840 Woodbridge in Topeka, Kansas were exposed to fall hazards of 10 feet or greater. Employees applying sheeting on the roof from a work platform positioned on the forks of the CAT TH63 telehandler were exposed to a ten (10) foot fall in that the 4' by 16' work platform was not secured to the mast or forks.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  Employees at a residential construction job at 5840 Woodbridge in Topeka, Kansas were exposed to fall hazards of 10 feet from the eave to the ground. Employees applying sheeting on the roof from a work platform positioned on the forks of the CAT TH63 telehandler and were not protected from falling in that the work platform did not have guardrails.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Employees performing a residential roofing job at 5840 Woodbridge in Topeka, Kansas were exposed to fall hazards of 10 feet from the eave to the ground fall protection was not used on the 8 in 12 pitch roof.
