29 CFR 1910.23(c)(1)(iii): Standard railings were not provided on every open sided floor or platform 4 feet or more above adjacent floor or ground level where there is equipment with which falling materials could create a hazard. Inside the "lowside" warehouse area, platform mezzanines used for storage of tires was not equipped with all elements of a standard railing (top rail, intermediate rail, and posts). An intermediate rail was not installed to help protect employees from materials that could create a struck-by hazard. The distance from the platform to the adjacent floor below is approximately 81 inches.

29 CFR 1910.23(c)(2): Runways four feet or more above floor or ground level were not guarded by standard railings (or the equivalent) and toeboards. Note: Toeboards are only required where tools, machine parts, or materials are likely to be used on the runway. Inside the "lowside" warehouse area, runways used to gain access between the platform storage mezzanines were not equipped with an intermediate rail to help protect employees from materials that could create a struck-by hazard. The distance from the runway to the floor below is approximately 81 inches.

29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have a vertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfaced throughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, or ramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard. Inside the "lowside" warehouse area, several sets of fixed industrial stairs used to access platform mezzanines were not equipped with an intermediate rail, exposing employees to a fall hazard. The fall distance from the platform to the floor below is approximately 81 inches.
