Wilbert Funeral Services, Inc., 432 Noble Rd, Girard, Pa 16417, on or about February 12, 2015, and times prior thereto: An employee voluntarily used an air purifying half mask respirators with cartridges (3M brand #7502 with 3M brand cartridges #60921) during spray operations in the employers spray application area of the facility. The employee was not provided with a medical evaluation prior to use of the air purifying half mask respirator.

Wilbert Funeral Services, Inc., 432 Noble Rd, Girard, Pa 16417, on or about February 12, 2015, and times prior thereto: An employee voluntarily used an air purifying half mask respirators with cartridges (3M brand #7502 with 3M brand cartridges #60921) during spray operations in the employers spray application area of the facility. The employer did not establish portions of a Respiratory Protection Program that covered medical evaluations and respirator maintenance and care.

Wilbert Funeral Services, Inc., 432 Noble Rd, Girard, Pa 16417, on or about February 12, 2015, and times prior thereto: An employee voluntarily used an air purifying half mask respirators with cartridges (3M brand #7502 with 3M brand cartridges #60921) during spray operations in the employers spray application area of the facility. Employees that voluntarily used half mask respirators were not provided with the information in Appendix D to this section. b.) Wilbert Funeral Services, Inc., 432 Noble Rd, Girard, Pa 16417, on or about February 5, 2015, and times prior thereto: Employees voluntarily used filtering facepiece respirators (dust masks) (Moldex brand 2300 N95) during mixing operations in the mixer area of the facility. Employees that voluntarily used filtering facepiece respirators were not provided with the information in Appendix D to this section.
