29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  Employees perform servicing and maintenance activities on equipment including, but not limited to the Amadan Punch Press; Amada CNC Brake Press, and the Mazak Laser Cutting machine which had multiple energy sources including energy sources such as but not limited to pneumatic, hydraulic, electric, gravity. Written machine specific energy control procedures for these machines were not developed and implemented.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:  West Welding area - Employees operating an unknown/unmarked Bench Grinder were exposed to tool jamming hazards in that the work rest on both sides of the operator was adjusted to 0.5 inch from the grinding wheel.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  West Welding area - Employees operating an unknown/unmarked Bench Grinder were exposed to struck by hazard in that the tongue guard on the right side of the operator was adjusted to 0.5 inch from the grinding wheel.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.215(d)(1): Spindle speed(s) on grinding machine(s) were not checked before mounting to assure that it did not exceed the maximum operating speed marked on the wheel:  West Welding area - Employees operating an unknown/unmarked Bench Grinder were exposed to struck by hazard in that the marking or tag of the grinder was missing, therefore they could not ensure that the maximum RPM rating of each abrasive wheel would be compatible with the RPM rating of the grinder motor.  29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.
