29 CFR 1926.502(d)(17): The attachment point of the body belt was not located in the center of the wearer's back. The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: On or about 5 February, 2015, at the above addressed jobsite, an employee was working from an aerial lift with a lanyard attached to his harness on his side right hip instead of his middle back, to prevent him from falling out or being ejected from the basket exposing the employee to a fall hazard of approximately 25 feet in height.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: On or about February 5, 2015, the employer did not ensure that the employee working in the JLG Aerial Lift was trained to operate the equipment, exposing the employee to a fall hazard of up to 25 feet in height.
