29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): (a) On February 2, 2015 the inspection revealed that the roll forming/bending machine was not provided with at least one of the following methods of point of operation guarding: a point of operation guard, a point of operation device, a point of operation awareness barrier (if a point of operation guard or device is not feasible), or an emergency stop device. The roll forming/bending machine was located in the middle of the shop bay at the time of the inspection.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips. (a) On February 2, 2015 the inspection revealed that employees were potentially exposed to serious injury from the unguarded blade on the Marvel band saw. The unguarded portion of the blade was located under the work rest of the saw, and the saw was located in the shop near the welding area.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least on half-hour. (a) On February 2, 2015 the inspection revealed that oxygen and acetylene cylinders were stored within about 3 feet of each other and were not separated by a fire resistant barrier, potentially exposing employees to serious injury in the event of a fire. The storage cages were located in the center of the work bay at 200 Lake Drive in Hammond, Oregon.
