29 CFR 1910.212(a)(1): Machine guarding was not provided to protect operator(s) and other employees from hazard(s) created by rotating parts and hot components of the machinery:     (a)  On or about January 29, 2015, an operator inadvertently entered the drag rollover machine in the mold department and was exposed to caught in between / burn hazards due no guarding being provided for the drag roll over clamp.  The drag rollover's clamp came down on the operators left hand resulting in amputated fingers and burns.     (b)  Operator(s) working at the cope rollover machine in the mold department were exposed to caught in between / burn hazards due to no guarding system being provided to prevent employee access to rotating parts and hot components of machinery.
